Brookvale Practice is a Community Wellbeing Practice!

Brookvale Practice is a centre for health and wellbeing!

What is a Community Wellbeing Practice?

A Community Wellbeing Practice is a GP Practice that has a community centred approach to healthcare. Taking a ‘glass half-full perspective’, it focuses on the positive factors and assets available in the local area that can improve your health and wellbeing.

Brookvale Practice offers a range of activities and initiatives that will help you improve your wellbeing. This ranges from drop in appointments to activity groups and events.

Each Community Wellbeing Practice has a dedicated Community Wellbeing Officer that works in the practice and local community. They will guide and assist you in improving your wellbeing. Brookvale's Wellbeing Officer is Catherine Jackman. You can contact Lyn Swift on  01928 589 799 or by emailing lyn.swift@wellbeingenterprises.org.uk
